The East Sussex C-Card scheme is a free and confidential co-ordinated condom distribution network for young people aged 13 – 24 years in East Sussex. It aims to provide quick and confidential access to condoms, supported by evidence based, accurate contraceptive and sexual health information, and sign posting to comprehensive contraceptive and sexual health services.

The scheme has been established in East Sussex for a number of years and currently includes providers from the following organisations/sectors: East Sussex County Council, local NHS organisations, pharmacies, GP surgeries, local colleges and schools, youth groups and other community and voluntary sector organisations.

Community Pharmacies and GP surgeries can sign up to provide the C-Card condom distribution scheme for young people by signing up to the Public Health Local Service Agreement (PHLSA) via East Sussex.gov.

All other services that are interested in providing the C-Card scheme, requests for training, resources and stock order forms should be made via email the C-Card mail box.

C-Card outlet providers quarterly monitoring

Safe practice and justification of the C-Card scheme relies on accurate completion of a small set of C-Card monitoring forms. Return of accurate monitoring information enables the continued supply of free C-Card products for your service.

Forms to be returned to C-Card coordinator on the first Friday following the end of each quarter:

  • C-Card registration quarterly reporting Excel form. Click here to download, you will find the form in your Downloads folder.

Note for PHARMACY C-Card providers only:

Please note that return and completion of quarterly distribution and registration monitoring forms is not required, as this information is entered directly onto the PharmOutcomes system.  Advice on use of PharmOutcomes is provided separately.

Note for GP C-Card providers only:

Please note that return and completion of separate quarterly distribution monitoring forms is not required, as this information is entered within the GP quarterly claim form. If providing the registration element of the service please complete and return the C-Card registration quarterly reporting Excel form
